In-Office Procedures
North Shore Gastroenterology offers a comprehensive range of in-office diagnostic procedures designed to identify digestive symptoms quickly and accurately. At our Westlake and Brooklyn locations, patients have access to noninvasive testing for conditions such as lactose intolerance, bacterial overgrowth, H. pylori infection, liver fibrosis, motility disorders, and hemorrhoids. These procedures allow our gastroenterologists to evaluate symptoms without the need for sedation or hospital-based testing, providing a convenient and efficient way to guide treatment and support long-term digestive health.
FibroScan Transient Elastography
FibroScan® transient elastography is a noninvasive, pain-free test used to measure liver stiffness, which helps detect liver scarring (fibrosis). It is commonly used to evaluate liver conditions such as hepatitis B and C, cirrhosis, alcoholic liver disease, and metabolic dysfunction–associated steatotic liver disease (MASLD). The test is performed similarly to an ultrasound and takes only 10 – 15 minutes. A small probe placed on the right side of your abdomen sends a gentle vibration through the liver, measuring how quickly it travels. Faster vibration indicates greater stiffness and more advanced scarring. Patients should not eat or drink for three hours prior to testing. Your doctor will evaluate your score and discuss your liver health with you.
Lactose Intolerance Test
Lactose intolerance testing is performed using a hydrogen breath test, which measures the body’s ability to digest lactose (the sugar found in milk). If the small intestine does not produce enough lactase enzyme, symptoms such as bloating, gas, cramping, and diarrhea may occur after consuming dairy. Prior to the test, patients must fast for 12 hours and avoid high-fiber foods the day before. During the test, a small lactose-containing solution is consumed, followed by breath samples taken every 20 minutes over three hours to evaluate hydrogen levels and determine whether lactose malabsorption is present.
Bacterial Overgrowth Test
Small-intestinal bacterial overgrowth (SIBO) occurs when excessive bacteria accumulate in the small intestine, affecting digestion and nutrient absorption. Symptoms may include bloating, abdominal pain, excessive gas, diarrhea, or unexplained weight changes. SIBO is also detected using a hydrogen breath test similar to lactose intolerance testing. After drinking a special test sugar, breath samples are collected at timed intervals to measure hydrogen and methane levels, which help identify bacterial fermentation in the small intestine. Results guide treatment options such as antibiotics or dietary modification.
Urea (H. Pylori) Breath Test
The H. pylori breath test is a convenient and accurate method for detecting active Helicobacter pylori infection in the stomach or duodenum. H. pylori is a common cause of gastritis and peptic ulcers. Early diagnosis helps prevent ulcers and long-term complications. Patients should avoid eating or drinking for four hours prior to the test. After an initial breath sample, a small amount of Pranactin®-Citric containing urea is consumed. A second breath sample is collected 20 minutes later. If carbon dioxide byproducts of urease activity are detected, it indicates an active H. pylori infection.
SmartPill (Pressure Cam)
SmartPill™ motility testing uses an ingestible capsule that measures pressure, pH, and temperature as it travels through the digestive tract. This test is used to evaluate motility disorders such as gastroparesis or chronic constipation. During the appointment, patients eat a SmartPill meal bar and swallow the capsule. Normal activities can resume after a six-hour fasting period. Over the next three to five days, the capsule collects data while passing through the GI tract. The patient wears a small receiver to gather the information, which is later analyzed by the gastroenterologist to assess motility patterns.
Liver Biopsy
Liver biopsy is offered at our Westlake location for patients who require tissue evaluation to diagnose or assess liver disease. A biopsy may be recommended when imaging or noninvasive tests such as FibroScan are inconclusive. During the procedure, a small needle is used to collect a tissue sample from the liver for microscopic analysis. Your provider will determine whether this procedure is necessary based on your medical history and diagnostic findings.

HemWell Hemorrhoid Treatment
HemWell therapy is an in-office, noninvasive treatment designed to relieve symptoms of internal hemorrhoids. Using low-frequency energy, HemWell treats the hemorrhoidal tissue without cutting, banding, or anesthesia. The procedure is fast, comfortable, and effective for patients experiencing rectal bleeding, discomfort, or swelling associated with hemorrhoids. Most patients return to normal activities immediately afterward.
