To Schedule An Appointment
Please
contact our Westlake office at (440) 808-1212 or our
Garfield office at (216) 663-7064 to schedule your visit.
Our appointment scheduler will ask you for the
following information:
- your full name
- home number
- work number
- date of birth
- social security number
- marital status
- mailing address
- primary care physician
name
- reason for your visit and your current insurance
carrier.
If your primary care physician already
performed any lab work or radiology tests, please be
prepared to tell us the institution at which these tests
were performed. This information is necessary
to ensure that your appointment is scheduled in a timely
fashion as well as allow us to thoroughly prepare for
your scheduled visit.
North Shore makes every effort to respond to patients
quickly. If we are experiencing high call volume
and you are routed to our voice mail, please leave your
name and the number where you can be reached. We
will make every effort to return your call within the
hour.
Please review our Office Policy for information about our relationship with our patients.
If this is an emergency, please hang up and dial 911,
or make arrangements to visit your local urgent care
center or hospital.
If this is your initial appointment, please print our registration forms, fill
them out, and carry them with you to your initial appointment.
Please also be sure to bring your current insurance
card as well.
For returning patients with changes to their personal
information, such as name, address, or insurance, please
fill out the registration
form again with only those changes and bring with
you to your appointment.
